You have had one or more federal student loans discharged because of a total and permanent disability. Before your financial aid eligibility can be determined, please complete one of the following options:
- For Federal Pell Grant, Scholarships, and other grant options only, complete the Borrower鈥檚 Acknowledgement on the form below (page 1 only).
- For Federal Student Loans, complete the Borrower鈥檚 Acknowledgement and Physician鈥檚 Certification on the form below (page 1 & 2). Your physician must fully complete page 2.
