Consent to Disclose Educational Records (FERPA Waiver)
This form provides consent for a third party (parent, employer, etc.) to view a student's
educational records. You may want to consider talking to your student about filling
out a FERPA waiver if they want you to be able to access their information.

Bill Payment Authorized User
Students can give others (parents, employers, etc.) the ability to access their account
information. In compliance with FERPA, student financial records may not be shared
with a third party without written consent. Adding an authorized user is written consent
that an individual may view account information and make payments on the student's
behalf. Please note that authorized users do not have access to their student's stored payment methods, academic records, or other
personal information.
Third Party Request for Student Information
Submit this form to the Office of the Registrar to request academic information for
a student. In order to be released, your student must have the "consent to disclose"
form on file.
Dependent Grade Form Request
Submit this form to the Office of the Registrar to request a semester grade report
for a student. The first page of last year's federal income tax return must be submitted
with this form. In order for information to be released, your student must have the
"consent to disclose" form on file.
