Data Request Form (DRF)

The Office of Planning & Analysis (OPA) serves as a central point for submissions of data requests (encompassing aggregate data, record level data and data for Qualtrics surveys) for active students pursuing approved research and employees who are in need of data related to their current job function.

Aggregate Level Data

Aggregate level data are summarized record level data that resides in our enterprise systems.  For example, I want a count of undergraduate students who have completed their degree in my program over the last three academic years with a breakdown of headcounts by race/ethnicity and sex type for each year.

Record Level Data

Record level data are data at the transactional and person level.  For example, I want a list of each undergraduate student who completed their degree in my program last year with their 蹤獲扦 ID, sex type and race/ethnicity.

Qualtrics Data

Qualtrics data are data utilized in our institutionally approved survey tool (Qualtrics).  You may use the link below to transfer to the 蹤獲扦s Qualtrics webpage where you can request a Qualtrics account, get assistance in creating a survey, or request contact data related to your prospective survey recipients.
